ACCOMMODATION/MODIFICATION REQUESTS
Students who have a disability/or disabilities that require a reasonable accommodation in the room assignment process or a modification in the room itself should connect with Student Accessibility Services. Students will complete an online application.
Documentation for an Emotional Support Animal or Housing Accommodations
Emotional Support Animal:
- Minimum documentation requirements:
- Verify the disability and/or disability-related need
- Verify the ESA provides support alleviating one or more symptoms or effects of a disability or disability-related need.
- From a licensed health care practitioner; or meet the needs stated by HUD
- The student must provide proof of compliance with state and local requirements for vaccinating the emotional support animal.
- In accordance with Florida State law (828.30), all dogs, cats, and ferrets (including Service Animals) are required to have current rabies vaccines.
Other Housing Accommodations:
- Minimum documentation requirements:
- Verify the disability and/or disability-related need for the accommodation
- Provides identifiable relationship between the requested accommodation or modification and the individual’s disability.
- To ensure accommodations in the room assignment process can be made, students should complete the Housing Contract and Student Accessibility Services process prior to:
- June 1 for the fall semester
- November 1 for the spring semester.
- Requests are accepted after this point, but assignments are based on availability.
Policy on Reasonable Accommodations and Assistance Animals in University Housing
Students have the option to utilize an SAS Verification Form as documentation for ESA, ESA Vaccination, and Housing Accommodation documentation
